Personal Health Plan Payment Options

Save paper and time by paying your monthly Personal Health Plan bill online! You can pay
by credit card, debit card or bank account withdrawal. Simply use the My Health
Plan login on the right of this page.
In addition to one-time payments, you are able to set up a recurring monthly
payment that deducts your invoice amount on your due date using the payment
method of your choice.

To make a one-time payment by check or money order, include your invoice and
nine-digit group number with your payment and please mail to:

Please Note: If you choose to pay your monthly payment through your bank's
online bill pay, please include your nine-digit group number in the payment
information. If your payment amount changes, please remember to update your
bank account with the new monthly amount.
