Provider Networks for Pennsylvania Members

A network is a group of hospitals, clinics, medical centers and physicians that participate with Consumers Life and its affiliates. Prior to obtaining services, you can ensure maximum cost savings and avoid unexpected charges by verifying the status of providers.

If you are a current member of Consumers Life, please select the logo that matches the one on your ID card. This will ensure you are selecting the appropriate network for provider participation in the designated service area.

If you have Devon ONLY on your ID card in PA, please select the Devon logo below. Use of the Devon providers will result in your maximum benefit levels. Please refer to your certificate booklet for benefit explanation.

If you have these three individual logos - SuperMed, Vantage, and Devon - listed on your ID card in PA, please select the Supermed/Vantage logo below. You are required to utilize providers from this directory in order to obtain higher benefit levels when seeking services within the Vantage service area. The Vantage service area is defined as the following counties:

Erie, Crawford, Mercer, Venango, Warren, Forest, Elk, McKean, Clearfield, Jefferson, Cameron, Potter, Clarion, Armstrong, and Lawrence.

Use of Devon providers within the defined service area will result in lower benefit applications. If you are outside of the defined service area, please select the Devon logo below. Use of Devon providers outside the Vantage Service area will result in a higher benefit level.

If you are not a current member of Consumers Life, and unaware of the potential networks available to you, please contact your Sales representative or Benefits Administrator for clarification.
